We require at least one full business day notice to cancel or reschedule an appointment.
Late cancellations or no-shows will result in a $75 fee billed to your account.
Three missed appointments may result in discharge from the practice.

These services can be scheduled after care has been established and appropriate medical history has been reviewed, never at an initial visit to establish care.

• Pre-operative clearance evaluations
• Motor vehicle accident (MVA) evaluations
