Patients arriving late may be asked to reschedule depending on the provider’s schedule. This helps ensure all patients receive the time and attention they deserve.
If you know you will be late, please call our office. In some situations, your visit may be converted to a telehealth appointment if appropriate.
Arriving significantly late and needing to reschedule will be considered a late cancellation and may result in a $75 fee.

Telehealth appointments may be available for certain visit types when appropriate. Please contact our office or check the patient portal to determine whether your concern qualifies for a telehealth visit.
Telehealth visits must comply with state licensing requirements. Patients must be located in a state where the provider is licensed during the visit.

Prescription refill requests should be submitted through your pharmacy directly. Many pharmacies offer online refill services through their websites or mobile apps.

Patients must maintain regular appointments with their provider to continue medication management, specifically to control hypertension, diabetes, mental health wellness management, ADD & ADHD, thyroid, etc. Most medications may require periodic review and follow-up visits.
Controlled medications are prescribed at the provider’s discretion and only when medically appropriate. Patients may be required to complete additional evaluations, monitoring, or documentation in accordance with medical guidelines and state regulations.
Yes, but it depends on the type of visit. Your provider will address the primary concern scheduled for the appointment. Additional concerns may require a separate visit to ensure adequate time for proper evaluation.
If you are being seen for an annual physical, you will need to be seen at a separate visit with your provider to address medical concerns such reviewing and refilling medications, test results, referrals, documentation etc.
Annual physical exams focus on preventive care and health screenings. Visits addressing specific medical concerns as well as reviewing and refilling medications require additional evaluation and are billed differently by insurance.
Insurance plans generally do not cover both a physical exam and treatment of additional medical concerns during the same visit, which can result in additional charges to the patient. Scheduling these services separately helps ensure proper billing, prevents unexpected out-of-pocket costs, and allows your provider the appropriate time to fully address each type of visit.
Yes. Please provide all required forms and documentation and bring them with you to your appointment so they can be scanned into your chart. They must be completed during your scheduled visit with your provider.
If more paperwork is required or unable to be completed during your visit, additional processing time and fees may apply. Please allow up to 7–10 business days for completion.
Our clinic does not participate in Workers’ Compensation (L&I) claims and we are unable to see patients for work-related injuries.
For Motor Vehicle Accident (MVA) injuries, patients must already be established with our practice and the initial evaluation for the accident must be performed by our providers. We are unable to assume care for MVA injuries that were first evaluated by another facility such as urgent care or the emergency department.
MVA evaluations are also not performed during new patient establishment visits and must be scheduled as a separate appointment.
